- Date and time
- Location (Where did it happen, city state)
- Type of incident (Trailer, Trail, Barn, Mud, etc.)
- Equipment used (Rescue Strap, Rescue Glide, ropes, crane, etc.)
- Injuries to the animal
- Total time to complete the rescue
- Outcome
